Many care homes are now required to take a look at their program and services at least once a year. Other Care homes will do a self assessment to see what areas can be improved to make their well run homes, even better! Whatever the reason, we have digitized "the DDS service quality assessment booklet" and made it easy to fill it out.

For only $4.95, you can use this program to make a Quality Self Assessment for your care home.

Here is what you do: 

• Sign up with our service and activate your account with the link provided in the email. If you have already created and activated the account, use the right portion of this page to sign in.
• You will be able to login to you account and pay for your care home. If you have more than one home, you will need to purchase one Assesment for each Home. Some of users may use this facility to complete assessment forms for other facilities as well.
• Each form is divided into 8 Steps.
• Each step contains 3 text areas where you can input your assessment based on the questionaire given to the right.
• One of the text area is your plan for the things to follow up. We tried to make this as easy and flexible as possible. 

At the end of the 8^th step a report will be generated from what you wrote in the text boxes.
